Following advice published in Connexion that the centre where one was vaccinated could change the Maiden name to married name my wife contacted our MT who did our jabs. He tried but was unable to change it. But he did write on the certificate my wife’s married name, signed it and put his stamp.

Had mine done at the pharmacist in maiden name Tried to get it changed she said no I took a French friend to point out my passport was in different name she still refused I phoned my CPAM quoting the article in connexion and they told me that they could not change it on my word in an english speaking paper and that it was rubbish it had to be my maiden name They suggested I carry a copy of my birth certificate with the pass port I then tried to argue the connexion point with the embassy advice and they said that it was French rules that matter not the UK embassy advice they then hung up on me I will try and ask the pharmacist to add my married name as an extra but have a feeling she will not budge
